Ann Fills Out an Accident Report

When Ann fell and broke her arm at work, she had to fill out an accident report. Her boss gave her the form and helped her fill it out. He told her it was very important to fill out the report completely. The form would be kept on file in case there were any questions about her workers' compensation claim.


Ann had to fill in every space. Some of the information was hard to remember. The accident had happened only a few days before. But it happened so quickly that it seemed like a blur in her mind.


The first parts were easy to fill in. Ann wrote today's date and the date of her accident. She will always remember that day!


The next space was a little harder. It asked for the time the accident occurred. Ann didn't remember the exact time. Her boss told her to write "approximately" and the time she thought it happened. Approximately means "about." So, Ann wrote "approximately 10 A.M."


Then Ann filled in her name and home address. She put down her social security number and her home phone number. Ann checked the box for female and wrote down her age.


The next box asked for Ann's job title. She wrote "file clerk." She had to think hard to remember the date she was hired to answer the next question.


The rest of the form was not as easy. She had to tell about the accident. Ann's boss told her to write as many details as she could remember.


The next box asked for the address where the accident occurred. Ann wrote down the address of the building where she works. Then she had to fill in the exact location of the accident. She wrote "near the sink in the ladies' room."
